All patients wishing to join the practice are invited to collect a Registraion Pack from reception. The Registration Application Forms enclosed need to be completed for each patient and returned to the surgery.  You will also need photo ID, evidence of address (bill or statement) and visa if appropriate.

We do not accept new patient registration requests on line although you can print off the forms (click on links below) and deliver the completed forms to the surgery with your supporting documentations.  Please note that all prospective new patients are required to complete both the GMS1 Family Doctor Services Registration Form as well as the relevant Health Questionnaire.

(Note:please print off as many times as required and complete a separate form for each adult and each child)

Please make an appointment for a new patient medical check.  Your medical records can sometimes take a while to follow you to a new practice and so this new patient consultation helps us to get to know your medical needs as soon as possible. 

Change of Personal Details
If you change your name, address or telephone number please remember to inform the surgery. As long as you are still within our catchment boundary, this may be done either by completing a Change of Details form, also available from reception.
